LASIK Coverage and Vision Insurance
Some vision plans include coverage for LASIK eye surgery. If a vision plan covers LASIK, coverage may be a flat amount or a percentage of the total procedure cost. You can view your plan's Lasik coverage by clicking on the Vision Insurance app from the homepage of Zenefits. The information is located under the Benefits Summary on the Vision Overview page. This will usually be found under a Laser Correction Surgery benefit.
Be sure to review the Limits and Exclusions section of the Summary of Benefits and Coverage to ensure that LASIK is a covered benefit, as well as any exclusions or exceptions to the benefit listed. For more information, reach out to the insurance carrier directly, or contact Zenefits Support.
