Which forms and documents need to be signed to finish my Zenefits Payroll setup?

To finish applying for Zenefits Payroll, you (or your designated signatory) will need to sign a few authorization forms that allow Zenefits Payroll to file and pay your Federal and state taxes.

The designated signatory for the account will sign:

  • Form 8865 (the Federal Power of Attorney form) to authorize Zenefits Payroll to automatically sign Federal returns and pay Federal taxes on behalf of the company.

  • Either a state-specific Power of Attorney or Electronic Funds Transfer (EFT) authorization to authorize Zenefits Payroll to file and pay state taxes. There's one form for each state where your employees live or work.

Power of Attorney forms authorize Zenefits Payroll for payroll and tax filing purposes only. No other legal authority is granted by the forms.

Signatories can digitally sign the forms in the Pending Signature list right in Zenefits. However, if you have employees who live or work in Alabama, Montana, New York, or Vermont, you'll need to download the Powers of Attorney forms in the Pending Notarization list, sign them, have them notarized, and then upload them to Zenefits.

If you designate someone else (e.g., an accountant) as signatory, they'll need to register in Zenefits, then log in and use this link to access the forms.
