Submit Zenefits Flex Benefits Claims Through the Dashboard
Each type of Flex Benefits account has a particular form for claims.
To locate the appropriate claims form in Zenefits, log in and select the appropriate benefits app (Flex Spending Account, Health Reimbursement Account, or Commuter Benefits) from the main menu. Once you've located the correct form, you'll select the Submit Claim option from the overview page.
You'll need to provide the following information to complete the claim filing process:
- Type of claim (e.g. Medical FSA / Dependent Care)
- Claim Recipient (e.g. Employee, Spouse, Dependent)
- Service Provider
- Date of Service
- Type of Service
- Amount
- Upload Receipts
Employees who no longer have access to their Zenefits dashboard, but are still within the claim submission window, can send the following forms to claims@zenefits.com.
