What happens to an employee’s Avidia Bank HSA when it's terminated?

When an employee is terminated in Zenefits, the HSA account will be re-associated to Avidia Bank the same day as termination.

They will not receive any further contributions after the termination date.

This process is similar to what happens to an employee’s HSA when an employer cancels the company’s HSA.

A new card will be issued and automatically sent to the employee. The employee will also receive a letter in the mail. Additionally, the HSA account number will remain the same.

The mail will have the following information:

Dear First_Name;

We have been informed by the Administrator of your HSA that you have experienced one of the status changes below:
· You are no longer covered by a High Deductible Health Plan
· You are now covered by another health plan or Medicare Part A or B
· You are being claimed as a dependent on another person's insurance policy or tax return

Due to your recent change in status, your remaining balance has been transferred to an Avidia Bank Individual HSA for your continued use. Your HSA account number remains the same.

In the next 7-10 business days, you will be receiving a new debit card to access your remaining balance. You will still have continued online access to your information prior to your status change but will need to set up new log in credentials for HSA access going forward. Until your new card arrives, you can set up online access and use the Portal Bill Pay Feature to make payments.

The employee will also receive the following email from Zenefits:

Your remaining HSA balance has been transferred to an Avidia Bank Individual HSA for your continued use. You will have continued online access, but will need to set up new log in credentials by visiting: https://www.mywealthcareonline.com/avidiahealth/. Please use employer code AVI2000  and employee code (your SSN) during the online registration process.
