Balance Billing for Insurance Claims

In balance billing, the provider bills a subscriber for the difference between the provider’s charge and the allowable amount. For example, if the provider’s charge is $100 and the allowed amount is $70, the provider may bill the subscriber for the remaining $30.

Providers may not use balance billing for services covered under the plan's SBC provided by an in-network provider.
