What is IRS Form 1095-A?

IRS Form 1095-A (Health Insurance Marketplace Statement) is the equivalent of Form 1095-C for individuals who purchased health insurance coverage for themselves (or anyone in their household) through the Marketplace (and not through an employer). Like Form 1095-C, Form 1095-A provides details on who was covered, what kind of coverage they had, and when they had coverage in the previous year.

If Form 1095-A indicates that the individual (and their family members) had coverage that met the shared individual responsibility provision for the entire year, they can check the full-year coverage box on their personal return. If not, individuals claim an exemption or make an individual shared responsibility payment.

Zenefits does not support or provide Form 1095-A to employees.
