What is IRS Form 1095-C?

IRS Form 1095-C  is a statement provided by an Applicable Large Employer (ALE) to each of its employees who were eligible for coverage in the previous year. The form helps the IRS enforce the ACA employer mandate by monitoring the type and cost of coverage offered to employees, and the number of employees who were offered this coverage.

Form 1095-C provides details to employees on whether coverage was offered, what kind of coverage they (and their dependents) enrolled in, and when they had coverage in the previous year. Employees can use this information to determine whether they can claim full-year coverage on their personal returns, or if they should claim an exemption or make an individual shared responsibility payment.

Additional notes

  • Small groups with fewer than 50 full-time employees will not be issued a 1095-C form. Zenefits only supports 1095-C forms for groups who are an ALE.
  • Employers must complete the ACA process in Zenefits to generate the 1095-C forms and then send the form to employees.
