What is IRS Form 1095-B?

IRS Form 1095-B is a statement provided by insurance companies, government agencies (e.g., Medicare), and self-insured employers to individuals who had health coverage for themselves or their dependents.

Like Form 1095-C, Form 1095-B provides details to employees on who was covered, what kind of coverage they had, and when they had coverage in the previous year. Employees can use this information to determine whether they can claim full-year coverage on their personal returns, or if they should claim an exemption or make an individual shared responsibility payment.

Carriers send 1095-B forms directly to enrollees, so brokers do not have access to them. If you did not receive your 1095-B form in the mail, you can get your form either through the carrier portal or by calling the carrier and requesting a new form.
