What contact information do I need to provide for my insurance application?
After an employee selects their plans, they will be asked for their contact information.
You’ll be prompted to provide:
- Your phone number
- Your home address
- A PO Box cannot be used for this address. Please see this page on Using P.O. Box for Residency Address for more information.
After entering your information, you’ll be prompted to enter contact information for your dependents, including:
- Their e-mail address
- Their phone number
- Their address
- If the dependent shares your address you can keep the Use my address box checked.
