Here's a list of information that might be needed about your dependent in order to complete an insurance application:
- Social Security Number: If a dependent does not have a Social Security Number, leave this field blank
- Address: No P.O. boxes allowed
- Date of Birth
- Name and Provider ID number of Primary Care Physician (PCP): this requirement only applies if you are enrolling in an HMO plan.
